[1] Take a new file of 800 pixels,200 pixels, of resolution 72 dpi in the RGB mode.
[2] Now take the Pen Tool create a shape.
[3] Make the selection and fill with any color would you like.
[4] Go to Blending Option use the following settings.

[6] Make the copy of previous shape layer.
[7] Create another shape with the help of Pne Tool.
[8] Make the selection and fill with Black color.
[9] Go to Blending Option use the following settings.

[11] Then take the Shape Tool and select this one.
[12] Create a shape and then select the Elliptical Marquee Tool use these settings.
[13] Create a shape over the previous shape see in the next image.
[14] Your image should be look like this.
[15] Go to Blending Option use the following settings.

[17] Create another shape with the help of Elliptical Marquee Tool and fill with White color.
[18] Go to Blending Option use the following settings.

[20] Create a new shape with the help of Elliptical Marquee Tool.
[21] Take the Gradient use the following settings.
[22] Fill the selection with the help of Gradient Tool.
[23] Now change the Blending and set the Opacity.
[24] Your should something look like this.
[25] Create a new shape with the help of Elliptical Marquee Tool.
[26] Take the Gradient use the following settings.
[27] Fill the selection with the help of Gradient Tool.
[28] Now change the Blending and set the Opacity.
[29] Your should something look like this.
[30] Create a small circle with the help of Elliptical Marquee Tool and fill with White color.
[31] Go to Blending Option use the following settings.
[32] Your image should be look like this.
[33] Make the copy of previous shape layer.
[34] Now take the Rounded Rectangle Tool.
[35] Create a shape and fill with Black color.
[36] Now go to Blending Option use the following settings.
[37] Your image should be look like this.
[38] Make the copy of previous shape layer.
[39] Now take the Text Tool create your text.
[40] Now finally you can fill the Background with Black color your final image is ready.
